I acknowledge and agree that these terms and conditions as well as any amendments thereof constitute a contract between Polaris Bank Limited and me and govern my membership in the Polaris Exclusive Club and the use of Polaris Visa Signature Card.

• I agree that acceptance into the Polaris Exclusive Club constitutes an acceptance to purchase a Polaris Visa Signature Card and once submitted, purchase will not be cancelled.
• A monthly fee of N2,000 is applicable to all Polaris Exclusive club members.
• I hereby authorize Polaris Bank Limited to debit my account for all fees incidental to membership and issuance of Visa Signature Card.
• I agree that neither this Card nor my membership in the Polaris Exclusive Club shall be used for fraudulent or criminal acts, money laundering, terrorism acts or any other illegal act.
• I agree that Polaris Bank Limited can rely on my instruction using my access code/password without having to check the authenticity of that instruction except where Polaris Bank Limited is clearly aware that the instruction is fraudulent and I will be responsible for all Card transactions except where I have informed the bank that the Card is lost or stolen prior to the transaction being consummated.
• I agree that it is my responsibility to keep my card/ access code or password secret and private to myself and not disclose the details to any third party as Polaris Bank Limited will not be responsible for any fraud or loss arising from the compromise of my password or disclosure of my Card information to any third party.
• I agree that it is my responsibility to ensure secure, safe and proper use of my Card as Polaris Bank Limited shall neither be liable for any unauthorized use of the Card nor be liable for any fraud committed on the card.
• I agree to report to Polaris Bank Limited any incident of loss or theft of card, suspected compromise of password or fraudulent act of third party relating to the Card immediately upon occurrence for necessary action.
• I understand and agree that Polaris Bank Limited shall not be responsible for the action or inaction of third parties connected with the use of this card.
• I agree that any dispute between me and any third party connected to this Card shall not affect the right of Polaris Bank Limited to charge my account for any fee or payment due to be made.
• Any duplication or misuse of a Polaris Visa Signature Card will be subject to cancellation.
• Polaris Bank Limited shall not be liable or responsible for any defect in the product/merchandise/goods or services purchased with the Polaris Visa Signature Card nor warrant fitness for purpose of such product or services.
• I agree and acknowledge that Polaris Bank Limited does not give any guarantee against any damage, defect, or misuse of the card.
• Polaris Bank Limited reserves the right to amend or withdraw the Polaris Visa Signature Card with reasonable notice to the customer.
• All transactions through Polaris Visa Signature Card are subject to applicable laws of the laws of the Federal Republic of Nigeria.
• All disputes arising out of any transaction pertaining to the use of Polaris Visa Signature Card, shall be subject to the exclusive jurisdiction of Nigerian courts.
